Senior Living Aid at California Mission Inn
In Los Angeles County, CalAIM Community Supports are provided through Medi-Cal Managed Care health plans like L.A. Care Health Plan and Health Net. Since the program is statewide, the most effective way to find local services is to contact your specific health plan or a regional coordinating body in the San Gabriel Valley, which includes Rosemead.
Since CalAIM services are voluntary benefits offered through a Medi-Cal Managed Care Plan, you must be enrolled in one of the participating health plans to qualify. Below are ways to get connected to CalAIM Community Supports.

The most direct way to access CalAIM Community Supports is by calling your health plan’s member services.

Use the Findhelp Portal
Use the search portal Findhelp: Health Net recommends the Findhelp portal to identify local CalAIM resources and submit a referral.

Your primary care provider can assess your needs and help you get a referral to the appropriate Community Supports.

Step 2
Schedule A Visit
Schedule a visit to explore our comfortable living spaces, discover our amenities, and see how we can meet your needs—our family advisor will also help arrange a wellness assessment with our director.
Step 3
Apply for CalAIM
Your family advisor will connect you with trusted partners who assist with the CalAIM application process. There is no cost to apply for CalAIM.
Step 4
Begin at Your New Home
Once your belongings are safely moved to your new home at California Mission Inn and you’re settled in, you or your loved one will be ready to enjoy all the comfort and peace of your new living space.
